Object Object Forms help

Trying to debug an issue with selecting a form in a page. When selecting the form all we get is object object error. Loading up firebug look like we are getting an object reference error from the forms.svc service but I see nothing in the logs to help me out. I created a new sf instance and don't have this problems. We have tried removing all the forms we have and creating new ones without any luck.

Any ideas on how to debug this further? I don't see any differences in files (dll or configs between a new instance and the current instance of sf) Both our sp1.
