4.1 and strange Blog (Module) Entry

Hey guys,

Another small problem.  This is now the screen I get when editing my blog entry, which also happens with Events, News, etc.  Note the Title, Content and Summary fields and then the odd display of text beneath the fields.

Also I've lost the ability to give unique labels to Title, Content or Summary, to display the instructional text beneath them, or to handle required cases.  If applied, all labels, instructional text and error messages appear below the boxes where the text appears now in the screen shot.

Any ideas?
