Simple Form submittal

I created a simple form in SiteFinity and I can't get the results to post over the web.  The form submits fine when ran from the SiteFinity Project Manager tool.  I'm on version 4.0.1098.0.   The error we are receiving is this...

Value cannot be null.
Parameter name: No dynamic type with name Telerik.Sitefinity.DynamicTypes.Model.sf_learnmore

sf_learnmore is the name of my form.  This works as expected when doing it through the SiteFinity Project Manager.  Just not through the actual website.  Is there a step that I'm missing?  I followed the instructions that were in the video of how to create a form.  Seemed simple enough.   Thanks.
