Wrong widget template loaded on one auto-generated page

I have a custom widget template that I created on a page and the page is used as the base for several auto-generated pages from a dynamic module.  There are 6 auto-generated pages that come off of the same page and every one of them load the correct template but I have one that loads the default template.  I also noticed that the load time is slower on that page mostly because of the "Telerik.Web.UI.WebResource.axd" file which loads in about 1sec on all the other pages but takes 6+ secs when the template fails to load.

Has anyone else seen this issue or have any ideas what could be causing it?  There is nothing in my content that is doing it because the auto-generated pages are from a dynamic module and I made all the data match exactly to one of them that was working.  Plus the template that is failing is the exact same on all auto-generated pages.  It's content doesn't change between the different pages.
