Upgrade to 9.2.6220.0 fail on Custom dynamic module - Sitefinity - Bugs & Issues

I have completed upgrade to 9.2 version using CMS project manager and add new reference to Visual studio project. I was able to re-build the project and run the website. Right now, everything is running however, during the upgrade our custom dynamic module has fail. It is one of ours custom form widget. and currently we are not able to use it. Screenshot attached and logs is below. Any help would be appreciated.
