Custom Url Format for Module Builder built Modules

Hi,

There is a great article on customising the URL format for modules here: 
www.sitefinity.com/.../custom_url_formats_for_sitefinity_content_modules.aspx

However this doesn't seem to be relevant for modules built with the module builder. How do we customise the url for these modules? I would like /[Country]/[State]/[Title] for example where Country and State are fields in the module.
