Subscribe functionality in regular form

My business user wants to create a custom form that has a checkbox which allows the customer to subscribe to a particular mailing list that is already setup in Sitefinity.  They want to capture all of the answers from the form inside forms as normal.  How can I create a custom form that works like a form, but also has a checkbox to subscribe to a mailing list?
