How to add more fields to Comment Module? - Sitefinity - Front- & Back-End Development

As default, there are a few fields in comment submit form like Name and Email. The thing is i want to custom this Comment Module as my purpose, like i want to have First Name, Last Name, Age (number) and the content of a comment.

What should I do? Is there any document or thread discuss about this?

There is a Feature request about that functionality to have custom fields for comments:
