An Item with the same key has already been added error messa

Hi everyone,

I have created a custom page widget and while executing the project I was getting “An Item with the same key has already been added” error.
Can someone please help me why this error is coming?

It doesn’t create any log file in the specified location.

Is it possible to check the issue? If you have time, please have a look at below files.
