Date Picker Control

I am new to Sitefinity , and I need to create a form with Date picker , there is no date picker within the controls , so I have followed below article

and I have been able to see the control , but i faced 2 problems :

1. Once the form is published , I am unable to edit the control or add any new control.

2. I am unable to see the form responses .

Please advise.
