Having 39 clients at Rackspace Cloud, it is a huge nuisance to have to go to another service provider and a much more expensive hosting solution, because Full Trust is required to run Sitefinity.
In fact, it has stopped my company from transitioning any production sites to Sitefinity up to now.

Hello Joe Page,

Sitefinity 3.x edition can work in Medium trust ( medium trust setup), but not all features are supported - Workflow, HealthMonitoring, setting SMTP settings for Newsletters module. However, we are testing Sitefinity 4.0 under medium trust and we will provide support for it.

FWIW: I use a VPS hosting service to get around this at the moment.
