Is there supposed to be a default .skin file within _EmptyProject ?

From the Sitefinity 4.0 Beta Designers Guide (Sitefinity_Designers_Guide_4.0_Beta.pdf):

"we recommend you to use the predefined single .skin file from the Empty Template Package, but it is also possible to create a separate file for each control. The Empty Template Package must always be your starting point for creating themes in Sitefinity."

Where is this predefined .skin file?  I cannot find it.

Hello ColdCold,

This text has left from an initial version of Sitefinity 4.0 Beta Designers Guide. We will remove it and update the download. Of course you can create your own .skin file and use it with one of the themes that we ship or with a custom one.
