From the video/instructions:

Create a blank Sitefinity 4.1 website project that will receive the data from your existing 3.7 project and go through the process of setting it up, up to the point where you can login to the backend with your admin user account

Question:  I run the new web 4.1 project, I am asked to setup a database.  Do I *NEED* to setup a new database or can I upgrade from my previous database when launching the new site?  Or am I going too far in the steps if I'm creating a new database?

Hello Travis,

You need to create your project by following the steps explained in the guide. You need to create a new database and specify its type also provide login details for your admin account. Then you can continue with to the other upgrade steps.
