Upload Images or Documents gives me a blank screen

DISREGARD - The problem was between the keyboard and the chair. Some how or another I uploaded every directory except ClientBin - just an FYI - most of the other features will work without it. Sorry.

I am using the following:
Windows 2003 - IIS 6
SF 5.0
Everything works fine on my local machine, but now that I have deployed I get the following page when I select "Upload an Image" or "Upload a Document".

I have doubled checked my IIS & MIME type settings and everything looks fine. Am I missing one step? See attached.
