Media Field Issue using Module Builder - Sitefinity 4.5

Hello,

We are using the Module Builder feature in Sitefinity 4.5 and builds image media fields in that custom module.

Now, I am trying to remove an image from an already published content, I press on the "Actions" button on the image, select "Remove" than publish (please review the attached screenshot).

However, when I opened this item again for edit I found that image has not been removed yet.

Any suggestions for this issue?
