Tracking downloads and using Forms

Hi All,
In the new website, we would like to track downloads. More specifically, I want to capture the customer name and phone number and also which white paper was downloaded. Has this been done before, and if so, what kind of customisation is needed (on a high level).

At the moment, we have placed a form in front of the white pages section. This is good because we know the Customer is interested, but we need to know about the download. Ideal would be to go straight to the download page without any form, then click on the link. Once the link is clicked, fill out the form, etc.

Any suggestions?
