500 Internal Server Error - New Custom Form Widget using SiteFinity Thunder

I am getting a few JS errors when I run a page housing a form that was created in SiteFinity.  I have added 2 custom form widgets to my project via SiteFinity Thunder.  The forms and their interaction, etc. work fine but upon inspection with FireBug I am seeing the following:

"NetworkError: 500 Internal Server Error - server.com/.../WebFeedbackFormSubmitButton.js"

The JS files that are reporting 500 errors are the .js files created when you create a new form widget. I have not edited or changed these files.

Validation on the form is not working due to the JS errors. Any idea on how to resolve this problem?

 Can you please share the code of your form widgets with us. There is little we can do based on the information given so far.
