Adding Custom Fields to eCommerce Product Ratings

Is there a way to add custom fields to product ratings? For example, we would like to add a "Bottom Line' field where a user can choose from a few options like 'Yes, I would recommend this product'.  Also a small 'About Me' field.

The ratings and reviews are part of the comments. There is no interface for adding custom fields to the comments and  there is no out of the box implementation to allow this.
