Custom Form

Hi,

I have custom From this form in which table it will stored. I want to write a custom query for this form to retrieve data ,how and where can we write the query.

It'll create a new table based on the name, you're just gonna have to search for it once you create the form... 

I always add  a prefix in my config to group them together in the DB
