Diagnostics module shows Served From Cache = No

While caching is configured the Diagnostics module shows "Served From Cache = No" in many (not all) cases, even randomly for the same page if called several times. It shows also lots of queries. But the page should come from cache in any case. Can I trust? What are next steps? Can you help?
