Cannot send email to next approver: email template not confi

HI Guys,

Getting Above written error when approving in sequential workflow. But mail is being received as per email template.

I have created an email template for this and made a send email trigger to run that, and assigned this to "Approve Or Reject" workflow Action. 

But still error is coming: Cannot send email to next approver: email template not configured for Action Approve Or Reject.

How to configure email template specifically for Action Approve Or Reject?
