'subscribe now' button takes user to a page on your Support portal. You can customize that page the way you want.

I think you can do this on the header/footer/sidebar and inject a few lines of script.
Something along the lines of:
If you need it to be specific to a single application.
Apart from that you can define your own link by using the sidebar
Subscribe Now
- Given that you hide the original link first

Martin's suggestion is certainly valid.
Another recommendation is to create a redirect in the target portal page.
