I set a extensibility menu "MyTools" (OE 11.4) with different icons for each entry. (customization editor / menu&toolbar entries)
The contents of the "MyTools" menu is consistent with this settings. But the problem is the toolbar that repeat several times the same icon.
This is an issue in 11.4, please report it.
